Step 1: Access the My-Policies.com Platform
To begin the process, first visit the website my-policies.com.
Step 2: Select from the Benefits Menu
On the homepage, locate and click the "Service" tab. From the available menu options, choose the "Tools" category.
Step 3: Enroll in a Group Program
After entering the "Tools" section, you will find the option "Enroll in Group Program." Click on this option to proceed with the employee's enrollment in the company's health plan.
Step 4: Enter Required Information
On the next page, you will need to provide the following details:
- Company HR Email: Enter the email address of your company's Human Resources department.
- Contract Selection: Choose the appropriate insurance policy that applies to you.
- Employee Email: Add the email address of the employee you wish to enroll in the plan.
Step 5: Submit the Online Application
Once you have entered the necessary information, click "Submit." An electronic application will be sent to the employee’s email. The employee will need to complete and submit the application to finalize their enrollment in the plan.
Enrollment of a Dependent in Group Health Plan
If you wish to enroll dependents in the group health plan, please send an email to [email protected]. In your email, kindly include the following details:
- The name of the primary member (the employee already enrolled in the plan).
- The names of the dependents you wish to enroll.
- The relationship to the primary member (e.g., spouse, children).
- The birthdate of the dependents.
Removal of a Dependent from Group Health Plan
If you need to remove a dependent from the health plan, please send an email to [email protected]. Be sure to include the date of removal to ensure the process is completed correctly.
